By signing this agreement, the student represents and warrants that they are in good health and do not have any physical or mental disability, impairment, or pre-existing condition that would prevent them from safely participating in the activities at 10th Planet Jiu Jitsu LLC. The student acknowledges the advice of 10TH PLANET JIU JITSU CLUB LLC, which recommends that all participants receive a physical examination prior to engaging in any strenuous physical activity, including those conducted at 10TH PLANET JIU JITSU CLUB LLC.
The student understands that the activities involved in the listed program are physically demanding, potentially violent, and may expose them to serious physical injury. The student acknowledges the inherent risks associated with such activities and agrees to participate voluntarily.
While 10TH PLANET JIU JITSU CLUB LLC has taken all reasonable precautions to ensure the safety of its members, the student understands and accepts that there are risks involved in the program.
The student agrees to immediately inform 10TH PLANET JIU JITSU CLUB LLC management if their health condition changes during the course of their membership, including any accidents, illnesses, or new medical conditions that may affect their ability to participate safely.
The student acknowledges that physical contact may be required or customary during the course of instruction at 10TH PLANET JIU JITSU CLUB LLC. The student consents to such physical contact by employees, other students, and authorized individuals as part of the training process.
10TH PLANET JIU JITSU CLUB LLC is not responsible for any personal belongings brought into or stored within the facility or locker rooms. While the club will assist in locating lost or stolen items, it is not liable for the replacement or damage of any personal property.
In consideration of the student’s participation in activities administered by 10TH PLANET JIU JITSU CLUB LLC, including those run by its owners, officers, directors, employees, agents, representatives, contractors, subcontractors, volunteers, and premise owners, the student (and, if applicable, their parents/guardians if under 18 years of age) hereby expressly waives, releases, and forever discharges 10TH PLANET JIU JITSU CLUB LLC and its staff from any liability for personal injuries or damages resulting from participation in the program. This includes injuries or damages sustained either on or off the facility premises, whether caused by the intentional or unintentional acts of management or staff, to the fullest extent permitted by law.
By signing this agreement, the student acknowledges that they may be photographed or filmed while attending 10TH PLANET JIU JITSU CLUB LLC. The student grants permission for 10TH PLANET JIU JITSU CLUB LLC to use any photographs, video footage, or video streaming for promotional, sales, publicity, and advertising purposes across all media, including the internet.
By signing below, the student acknowledges that they have read, understood, and agree to abide by this waiver of liability. The student further affirms that they consent to the use of their image as described above.
